REGISTRATION FORM

Interested participants must submit the accomplished registration form on or before October 4, 2025

The registration form is available in the following formats:​

​

  1. Fill out the registration via Google form here.
  2. Download the .PDF format for painting category here and e-mail the accomplished form at icoandletycruzartcompetition@gmail.com with the subject heading Ico and Lety Cruz Art Competition 2025.
  3. Get the printed copy at the Museo ng Baliwag and accomplish or submit it in person.
